GP system version – Patient Access

This practice currently offers patients’ facilities to book, view, amend, cancel and print appointments online.

This practice currently offers the facility for all patients to order online, view and print a list of their repeat prescriptions for drugs, medicines or appliances.

We also now provide access to coded details in your clinical record (following the completion of the correct procedures).

What are detailed coded records ? this is clinically coded information which includes current active and past problems such as a diagnoses, test results e.g.; bloods results, peak flow, urinalysis, X rays. You will also be able to view any recorded heights, weights, blood pressures and smoking status.

To be able to view your clinical coded data you need to register for on-line access. This can be done by completing a form available at reception and bringing photographic ID.

IMPORTANT INFO FOR CURRENT ON - LINE USERS - those of you who already have on-line access are required to complete a new registration form. We are aware this is a little onerous given that you have already supplied this information, however this is an external system and this is required for confidentiality and security purposes.

Please use this application form: Patient online access form and leaflet
